Dr Neels Du Toit

MBChB (UFS, RSA), DipSEM FACSP FIPP Sports & Exercise Physician and Interventional Pain Proceduralist

I completed my undergraduate degree in medicine at the University of the Free State, South Africa in 1997 before coming to Australia in 2003. I then commenced specialist training as a sports physician at the Brisbane Orthopaedic and Sports Medicine Centre, and subsequently at Olympic Park Sports Medicine Centre in Melbourne. I completed my fellowship as a Sports and Exercise Physician in 2008. I went on to develop a special interest in chronic musculoskeletal pain conditions especially chronic spinal pain, chronic groin and shoulder pain.

I sub-specialised in interventional pain management completing all the modules of the Spine Intervention Society and was awarded a fellowship from the World Institute of Pain as a Fellow of Interventional Pain Proceduralists. Over the past 10 years I have spent most of my time treating chronic musculoskeletal conditions. I am passionate about making an accurate diagnosis and then providing a management plan allowing patients to return to a better life. Restoration of movement in my opinion is the key to treating chronic pain conditions. I perform all diagnostic and therapeutic pain interventions for chronic pain conditions including neuromodulation to manage chronic neuropathic pain. I also provide assessment of acute pain presentations, specifically providing early epidural interventions for acute radicular pain.

Dr Buchanan's opinion is that a whole person approach delivers the best outcomes in the framework of an interdisciplinary biopsychosocial approach. Dr Buchanan uses evidence-based interventional tools (when appropriate) to make the best diagnosis of the biomedical source of pain, which then informs decision-making for guiding treatment with the aim of minimising medication usage and supporting physical and psychological reconditioning so as to restore function as much as possible.

Dr Simon Cohen

BSc MBChB FRACP FFPMANZCA Specialist Pain Medicine Physician & Paediatrician

Dr Simon Cohen is a staff specialist in pain medicine at Monash and Monash Children’s hospital in Clayton, Melbourne. He is dually qualified in pain as a Fellow of the Faculty of Pain Medicine and in paediatrics as a Fellow of the Royal Australasian College of Physicians. He is an honorary lecturer at Monash University and past chair of the paediatric special interest group of the Australian Pain Society.

As a pain specialist he treats both adult and paediatric patients and can manage chronic pain secondary to injuries or medical conditions. There is also the challenging issue of the diagnosis of Complex Regional Pain Syndrome (previously known as causalgia or reflex sympathetic dystrophy). This persistent pain condition usually arises out of initial injury to a limb but becomes the major source of pain, suffering and loss of function despite expected healing of the original injury.

Specialist pain advice is of diagnostic value and also provides expert recommendations on treatment requirements. Pain management is holistic in nature and involves appropriate medications, allied health management in the form of physiotherapists and psychologists. It may also involve interventions such as nerve blocks or technologically advanced, treatments of peripheral or spinal cord stimulators.

One other area of expertise is the use of opioids for the management of chronic pain. The issue of long term opioids is topical and very much in the media currently regarding concerns of long term harms. Medical opinion is complex and requires knowledge of the evolving literature.
